Notarization of Parental Consent Form for U.S. Passport Applications for Minors

All minors under 14 years of age must now appear in person at a U.S. Embassy or Consulate when applying for U. S. passports whether for a new passport or for renewal of a passport previously issued. Please see the February 2004 article, " NEW U.S. PASSPORT REQUIREMENT FOR MINORS" under Citizenship Committee.

Signatures by both parents are required on passport applications for children under 14 years of age. The signing is made in front of a Consular Officer at a U.S. Embassy/Consulate. If one parent cannot accompany the child to the Embassy, then as of November 1, 2004 a signed Statement of Consent (Form DS 3053) granting permission for passport issuance for the minor child must be NOTARIZED. The absent parent should also provide ID.
